Request approval to accept the resignation of Adam Tinken from the position as Assistant Boys Volleyball Coach effective for the 2016-2017 school year.

Request approval to hire Ryan Skendzel for the position as Head Boys Volleyball Coach for the 2016-2017 school year. Mr. Skendzel will be paid $17% of BA-Step 6 ($59,886) or $10,181 based on the Collectively Bargained Agreement.

Request approval to hire the following individuals as instructors for the College Entrance Exam Program for the 2016-2017 school year. They will be paid the excess time rate pay of $36.09 per the Collectively Bargained Agreement:

Request approval to hire Allison Hornung for the position of Substitute Physical Education teacher for the second semester of the 2016-2017 school year pending successful completion of employment requirements. Ms. Hornung will be replacing Ms. Eliadis beginning on or around January 9, 2017 to on or around April 10, 2017 while she is on a Board approved Family Medical Leave of Absence (FMLA.) Ms. Hornung will be paid $22 per period.

Request approval to hire Libia Perez for the position as part-time Utility Clerk for the 2016-2017 school year pending successful completion of employment requirements. Ms. Perez will work two (2) hours per day at the rate of $17.40 per hour based on the Collectively Bargained Agreement.

Request approval to hire the following individuals as Pro-Techs to be objective music audition judges pending successful completion of employment requirements. These expenses have been budgeted for in the 2016-2017 school year:

Request approval to add a course overload for Zachary Collins, English teacher, for the second semester of the 2016-2017 school year due to increased enrollment. Mr. Collins will be paid an additional amount of $4,302.48 (15% of BA - Step 1) based on the Collectively Bargained Agreement.

Request approval to move David Cook, Third Shift Custodian, from probationary status to regular full-time status with full vested rights in all programs based on the Collectively Bargained Agreement.

Request approval to extend the Family Medical Leave of Absence (FMLA) for Lauren Evoy through the end of the 2016-2017 school year. Mrs. Evoy intends to return at the beginning of the 2017-2018 school year.

Request approval for Megan Fitzgerald to take a Family Medical Leave of Absence (FMLA) beginning on or around April 16, 2017 with a return on or around October 26, 2017.

Request approval to contract with Bulldog Solution Inc. for the second (2nd) semester of the 2016-2017 school year. The cost of the program for second semester is $9,216.20. The program will be funded through the Title 1 Grant.

Request approval for the Chicago Fire to use the Frank Stout Field House (C and S courts) on January 12th, 19th and 26th; February 2nd, 9th, 16th and 23rd and March 9, 2017 from 6:00 p.m. until 9:00 p.m. The organization will be charged a rental fee of $100 per hour for twenty-four (24) hours or $2400. A current certificate of insurance will be required prior to use.

Request approval for Tri-Town Little League Baseball to use the Frank Stout Field House (courts C and S) on Sundays from January 15, 2017 to April 9, 2017 from 4:00 p.m. until 6:00 p.m. to practice their baseball skills. The recommendation is no charge to the group for use of the facilities pending a roster of participants verifying 75% residency prior to use. A current certificate of insurance, which expires 5/5/17, is on file in the Activities Office.
